SUP TOURS at Pacific Sutera Beach LIABILITY WAIVER FORM

I agree that I have voluntarily chosen to participate in this SUP experience with Borneo Paddle Monkeys, and I understand that this activity, like most outdoors and water-related activities, involves inherent risks and potential dangers, including but not limited to the following:
– Risk of injury from physical activity, including but not limited to falls or contacts with other participants or boards.
– Risk of drowning or other water-related accidents.
– Risk of injury from board failure or malfunction, weather conditions, including but not limited to wind, waves, and lightning, or marine life hazards.
– Risk of injury from travel to and from the board location.
– Risk of injury due to my own actions or inactions.

I agree to comply with all safety rules and guidelines from Borneo Paddle Monkeys, and to all instructions from my guide, including the use of appropriate safety equipment such as my board leash and life jacket.

I acknowledge that I have been informed of these inherent risks and voluntarily assume all such risks associated with participating in this SUP activity. I knowingly choose to participate in this activity, knowing the potential dangers involved. I accept all responsibility for my own actions and my resulting injuries or damages.

I hereby release and hold harmless Borneo Paddle Monkeys, its owners, employees, agents, and affiliates, from any and all liability, claims, demands, or causes of action, arising out of or in any way connected with my participation in this SUP activity. This includes but is not limited to, any claims for personal injury, property damage, or wrongful death that may arise as a result of my participation.

I have carefully read this waiver and release of liability, fully understand its contents, and voluntarily sign it as my own free act and deed. I voluntarily sign this form of my own free will, fully aware of its legal consequences. I acknowledge that this agreement is legally binding and that I am giving up substantial legal rights.
